About Kingfisher PCN: Kingfisher Primary Care Network (PCN) is a collaboration between 5 GP partnerships (across 6 sites) in Redditch Town with a shared population of 58,955. Kingfisher has an overarching ambition to innovate general practice and build a sustainable model for general practice for the future.
Job Purpose: An exciting opportunity has arisen for a degree qualified nurse to join the Kingfisher Frailty Team in Redditch. This post will give you the rewarding opportunity to predominantly provide comprehensive assessments to patients in their own home and occasionally visit residents in care homes.
We are an extremely supportive team, and the successful candidate will work under the clinical supervision of the Frailty Team ACPs and GPs.
The team base is at Maple View Medical Practice and the core hours will be Monday to Friday 0800-1600 hours, which aids a good work-life balance. On occasion you may be required to work additional hours to suit the needs of the team.
Main Duties of the Job:

3. Use initiative and take responsibility for the quality of work and service provided to patients.
4. Act as a source of nursing expertise, knowledge, and skills in accordance with the NMC scope of practice.
5. Undertake clinical assessment of patients in their own homes or registered Care Homes with complex and/or chronic disease presentations.
6. Assess high-risk patients to reduce unplanned admissions and A&E attendances.
7. Be professionally accountable for the assessment, planning, implementation, and evaluation of care.
8. Maintain records as an autonomous practitioner liaising closely with the frailty team ANPs and patients' medical practitioners.
9. Work collaboratively with clinical colleagues to develop integrated care services.
Job Responsibilities:
Clinical:
1. Participate in the clinical assessment of patients with complex health care needs.
2. Support care management plans with pharmacological and non-pharmacological treatments.
3. Prioritise health care needs and refer for diagnostic investigations.
4. Develop, implement, and evaluate individual care plans with patients and their caregivers.
5. Participate in MDT discussions to facilitate high-quality care.
6. Independently prescribe and review medications.
7. Work within appropriate legislation and best practice evidence.
8. Initiate and review ReSPECT documents as required.
9. Demonstrate critical thinking and analytical skills.
Administrative:
1. Collect, collate, evaluate, and report patient information, maintaining accurate records.
2. Input data daily on EMIS patient administration systems.
Managerial:
1. Monitor health, safety, and security in the community.
2. Participate in root cause analysis for clinical/quality issues.
3. Identify innovations for service improvement.
Training, Research, and Development:
1. Provide education and development opportunities for others.
2. Develop your own clinical knowledge and participate in continuous development.
Personal/Professional Development:
The post-holder is required to maintain membership of an appropriate professional body and participate in training programs as part of this employment.
