You will provide primary support to General Practice staff with regards to prescription and medication queries. You will help support the repeat prescription system (including EPS), deal with acute prescription requests, structured medication reviews and medicines reconciliation on transfer of care. You will support systems for safer prescribing and provide expertise in clinical medicines advice needs of patients in our single practice PCN. Ideally you would have expertise in a chronic disease area eg diabetes or asthma management and could run chronic disease clinics. You will have qualified from an approved 18-month training pathway or equivalent and be able to practice and prescribe safely and effectively in a primary care setting (for example, the CPPE Clinical Pharmacist training pathways. You will be a qualified prescriber. You will be a qualified independent prescriber. Hours: We are looking for a clinical pharmacist to work full time hours 37.5 hours per week
