Community Nurse - Redruth Community Nursing Team

You will be based in the beautiful North Kerrier area with the Redruth Community Nursing Team. This area encompasses Portreath, St Day, Four Lanes, and Pool. We are looking for a caring, motivated, and dynamic community nurse to join our large, busy, professional team. We cover several GP practices in the Redruth and surrounding area. We want you to enjoy the experience that community nursing offers and how empowering people in their own home feels so very rewarding. Your role will require close collaboration with GPs, health and social care colleagues, including therapists and voluntary organisations. You should possess a commitment to team working and embrace innovative delivery of excellent standards of care. A real enthusiasm for community nursing as an autonomous practitioner and a desire for ongoing professional development is essential.
You should also have an understanding of the changing climate of care delivery in the community.
The ability to travel independently across the West Locality to support other teams and to fulfill the requirements of the post is essential; therefore, a valid driving licence will be required to undertake the role of a community nurse.
We work a 2-shift model to include 9 to 5 and 2 to 10 shifts, which includes working weekends.

Main Duties of the Job
Following an initial induction and ongoing clinical supervision, you will be responsible for:
1. Assessment, planning, implementation, and evaluation of care, without direct supervision, to patients, carers, and relatives within their home environment.
2. Working in partnership with patients and carers to offer support and advice and promote informed choice.
3. Supporting the role of the Community Nurse Team leader.
4. Working autonomously, providing effective, evidence-based care in line with competency and training records and organisational policies, procedures, and guidelines.
5. Supporting all locality teams by working in a flexible way, moving to work with the team in most need on a daily, weekly, or monthly basis as service provision demands.

Person Specification
Education and Qualifications
* Registered Nurse (Adult)
* Evidence of recent relevant professional development
* To act as a mentor for student university placements. Having the mentoring qualification or being willing to work towards this qualification and attending mentorship updates as required.
* Current Professional Registration - NMC
* Hold a post-registration diploma or be working towards a degree
Skills and Aptitude
* IT skills to include use of Word, Excel, email, internet, recording systems
* The ability to provide skilled, evidence-based nursing care to a wide range of patients
* Ability to deal with patients and carers who are under stress.
* Awareness of and adherence to policies including lone worker and manual handling policies
* Current knowledge of developments in nursing and healthcare in general.
