Job overview
The post holder will deliver, or advise on the delivery of, high quality safe nursing care, ensuring consistently high standards of practice and clinical excellence which comply with the Nursing and Midwifery Council (NMC) code of professional conduct.
To provide a comprehensive assessment and liaison service to people who are experiencing mental health difficulties who attend the Emergency Department (ED) and who are admitted to the Clinical Decision Unit (CDU) of UHL.
To provide assessment and advice on treatment of people who have been admitted to the medical, surgical and obstetric wards at UHL who are experiencing mental health difficulties.
To provide assessments to people who present with co-morbid presentations of either substance misuse or physical issues, and mental health difficulties in the ED and wards.
Main duties of the job
1. To recommend and set up discharge plans and enable users to access appropriate ongoing care or treatment from community services.
2. To provide advice and collaborative management plans on issues involving mental health to UHL ED and ward staff.
3. To support UHL colleagues in carrying out complex capacity assessments.
4. To provide an outpatient clinic service, which provides an assessment and signposting service for any patient receiving treatment at UHL who is unwilling to engage in traditional mental health community services or does not easily fit criteria for another service. This will predominantly be for persistent medically unexplained physical symptoms, and long term physical conditions where comorbid with common mental disorder. The aim will be to engage people in appropriate local mental health services, or national services, suitable to their needs.
5. It is expected that you carry out holistic biopsychosocial assessments and formulate plans of care to those who present in crisis.
Working for our organisation
The Lewisham Core 24 Mental Health Liaison Team (MHLT) is a 24 hour, 365 days a year assessment, consultation and liaison service, consisting of a multidisciplinary team of psychiatrists, psychiatric nurses, psychologists, dual diagnosis practitioners, occupational therapists and administrative staff, employed by the South London and Maudsley NHS Foundation Trust and commissioned by Lewisham Clinical Commissioning Group. It is part of the Psychological Medicine Clinical Academic Group (CAG) specialty in SLaM. The Core24 MHLT covers all mental health referrals from all teams, including ED and wards within UHL for adults (18 years and above).
You will be based at University Hospital Lewisham, Lewisham High Street, Lewisham, London, SE13 6LH.
Detailed job description and main responsibilities
1. Provide coordination and management of all referrals to the team, prioritising response times dependent on the nature of the referral.
2. Be required to make clear decisions, based on an analysis of complex presenting problems and judgement about available options. This will include those related to the admission and discharge of patients.
3. Utilise a wide variety of clinical skills appropriate to the needs of individual patients and the clinical setting in which they are required to be seen.
4. Carry out robust assessments, care planning and discharge planning. Have an ability to engage challenging and/or acutely distressed patients in potentially hostile and/or antagonistic situations.

* Practice Assessor Qualification
* Evidence of continued professional development
* Experience of working in liaison services
* Physical Health Qualification
* Additional training in relevant area
* Formal training in risk assessment and management
Skills and Experience
* A sound understanding and experience of how to assess and manage crisis within acute mental illness.
* Demonstrate skills in autonomous, senior decision making regarding discharge and signposting of service users following a comprehensive assessment.
* Evidence of skills working with the specific diagnosis of Personality Disorder, both in assessment and treatment.
* Ability to demonstrate knowledge of the epidemiology, presentation and immediate management of deliberate self-harm.
* Experience of delivering training to different disciplines.
* Able to use initiative and support other staff within the MDT.
* Ability to work under pressure.
Knowledge
* Significant post-qualifying experience in mental health services both inpatient and community based.
* Experience of working with service users with harmful substance misuse and alcohol issues.
* Experience of service development, Quality Improvement, Audit and leading change.
* Good communication skills both verbal and written.
* Experience of managing difficult situations demonstrating excellent communication skills.
* Experience of working with patients who have social issues.
