Role: Adults Social Worker - Hospital Discharge Team Location: Royal Shrewsbury Hospital & Princess Royal Hospital Rate: Up to £38 Eden Brown synergy are currently recruiting for Shropshire County Council who are looking for and Adults Qualified Social Worker to join their Hospital Discharge Team. This is a full time, locum position to start as soon as possible based full time at the hospitals. Paying up to £38 an hr. As part of the Hospital Discharge Team at Princess Royal Hospital and Royal Shrewsbury Hospital, your work will focus on improving patient outcomes by facilitating timely and effective hospital discharges while adhering to the principles of the Care Act (2014). The model we support aims to get people home as soon as possible to improve length of stay, enhance flow in the hospital and as a system, support the hospital to have the best performance and quality for the people who need acute intervention. To apply for this role you must: - Be a Qualified Social Worker with current registration with Social Work England (SWE). - Have proven experience in hospital discharge planning or working within a similar environment. - Have strong understanding of the Care Act (2014) and related policies. - Have the ability to work collaboratively in a fast-paced, multidisciplinary setting.
