As part of the multi-disciplinary team, you will screen individuals attending the Emergency Department, commence holistic assessments using a frailty assessment tool, and clinically reason a safe discharge or admittance plan with a focus on early intervention and a Home First pathway.

Main Duties of the Job
The successful candidate will provide high-quality, safe, and effective Occupational Therapy to patients attending the Emergency Department, Emergency Assessment Unit, and Short Stay wards. This includes assessing the needs of individuals attending the department, promoting early intervention, conducting Frailty specific assessments, and discharge planning. Support for the wider team will also be required, depending on service priorities.
You will work collaboratively with the wider multi-disciplinary team, undertaking joint assessments, liaising with families and places of residence, and providing follow-up treatment and handover plans to inpatient or community teams.
Support from senior members of the team will be provided, and education, training, and development will be promoted. Active participation in quality improvement, clinical audit, and research projects will be encouraged to deliver relevant and best practice.
A seven-day service for Frailty and Emergency care will be required, and flexibility to support colleagues on other wards will be encouraged.

Clinical Responsibilities
1. Enact professional and legal responsibilities for all aspects of clinical practice.
2. Carry out comprehensive assessments for individuals attending the Emergency Department and on inpatient wards, including using frailty and risk assessment tools.
3. Formulate and deliver comprehensive treatment, rehabilitation, and discharge plans based on clinical skills and evidence-based practice.
4. Evaluate patient progress on inpatient wards against agreed treatment goals and modify plans accordingly.
5. Determine patients' mental capacity and gain informed consent for treatment.
6. Maintain accurate written records using an electronic patient record system.
7. Determine and mitigate risks of patient and staff harm using agreed risk assessment frameworks.
8. Demonstrate physical ability to carry out Occupational Therapy assessments and interventions.
9. Represent Occupational Therapy at multi-disciplinary team meetings.
10. Contribute to the provision of a seven-day service.
Leadership and Management
1. Be responsible for the competent use and maintenance of rehabilitation equipment.
2. Assist in the supervision, teaching, and appraisal of junior staff and assistants.
3. Undertake delegated tasks and roles from the team lead.
4. Plan and organise own time, patient caseload, meetings, and training.
5. Assist in planning junior staff, students, and assistant timetables.
Education and Training
1. Organise and contribute to the team's training and education programme.
2. Contribute to the education of multidisciplinary groups, patients, families, and carers.
3. Supervise individual undergraduate students and facilitate practice-based learning.
4. Maintain a portfolio record for continuing professional development.
5. Participate in quality improvement and clinical audit programmes.
6. Implement and maintain evidence-based practice through continuing professional development.
Person Specification
Qualifications
* Diploma or degree in Occupational Therapy
* HCPC registration
Experience
* Experience in frailty and/or Emergency Medicine
* Experience of MDT working
* Experience of discharge planning including liaison with various health and social care providers
* Evidence of participation in training, audits, or quality improvement projects

Salary: £37,338 to £44,962 per annum pro rata
