Shakespeare Road Health Centre are looking for an experienced, motivated Advanced Nurse Practitioner to work alongside our friendly experienced team of clinicians and non-clinicians. The role is for 15 hours per week over 3 days - Monday 9am - 12pm, Tuesday 9am - 6pm and Thursday 9am - 1pm.
Main duties of the job
* Manage patients presenting with a range of acute medical conditions
* Implement and evaluate individual specialised treatment plans for chronic disease patients
* Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on the patient's health
* Request pathology services as necessary
* Process and interpret pathology and other test results as required
* Maintain accurate clinical records
* Prioritise health issues and intervene appropriately
* Recognise, assess and refer patients presenting with mental health needs
* Extended and supplementary prescriber
* Contribute to practice targets (QOF etc.) complying with local and regional guidance
* Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
* Support the clinical team with all safeguarding matters, in accordance with local and national policies
About us
Shakespeare Road Health Centre is a 2 site GP Training Practice in Rotherham, South Yorkshire. The practice has a list size of 5,800 patients. The practice team consists of skilled, multi-disciplinary staff with 2 GP Partners, 1 Long-Term Locum GP, 1 Advanced Nurse Practitioner, 1 Practice Nurse, 2 Nurse Associates. The clinical team also includes 1 Clinical Pharmacist, 2 Pharmacy Technicians, Mental Health Nurse and a Dementia Admiral Nurse. We have a management team, a reception/administrative team who provide excellent support to the clinicians.
The practice formed in 1987. We are high QOF achievers and participate in a range of enhanced services. We are a SystmOne practice and we have good relations with our neighbouring practices and are part of a 7 practice successful Primary Care Network.
Job responsibilities
* Develop, implement and embed health promotion and wellbeing programmes
* Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
* Implement and evaluate individual specialised treatment plans for chronic disease patients
* Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
* Provide wound care
* Provide specialist clinics
* Undertake the collection of pathological specimens
* Request pathology services as necessary
* Process and interpret pathology and other test results as required
* Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
* Maintain accurate clinical records
* Ensure SNOMED CT codes are used effectively
* Maintain chronic disease registers
* Support the team in dealing with clinical emergencies
* Recognise, assess and refer patients presenting with mental health needs
* Implement vaccination programmes for adults and children
* Deliver opportunistic health promotion where appropriate
Person Specification
Qualifications
* Registered Nurse NMC
* Must hold a digital badge or be able to obtain before start date
Experience
* Experienced ANP
* Working in General Practice is a must
* Experience in the assessment and management of minor injury and illness presentations
* Evidence of both team and autonomous working
