This is a permanent part time position 29.7 hours. (Please note the salary is Pro Rata for part time hours.)
The shift pattern for this position is 4/7 8.30am – 4.30pm.
An exciting opportunity has arisen for a Physiotherapist to work within an interdisciplinary Community Rehabilitation Team providing rehabilitation for patients in their own home. The post involves working as an autonomous practitioner within a multi-disciplinary team to complete comprehensive holistic MDT and physiotherapy assessments.
You will be responsible for planning treatment and care and supporting hospital discharges and admission avoidance referrals. The postholder should have excellent communication skills and be able to liaise with patients, carers, team members and other agencies to coordinate and support the patient through the service.
Experience in Adult Rehabilitation, Neurology, Orthopaedics, Medicine or Care of the Elderly Specialties is desired. Car driver essential and Community Experience would be beneficial.
