Requirements for the role:
* Experienced adults' social worker with mental health expertise and proficiency in Liquid Logic.
* Work collaboratively across disciplines, promoting a needs-based and partnership approach.
* Develop support plans, complete assessments, funding applications, and legal reports (CoPDoL, DoLS, 21a, court reports).
* Engage individuals with complex needs and support their families and carers under the Care Act.
* Assist young people transitioning into adult mental health services.
