The National Health Service (NHS) has developed a number of sub-acute services in recent years, recommending NHS trusts deliver more care closer to a person’s home rather than in acute hospitals. As pressure on emergency departments grows, new care models will be pivotal in delivering and relieving some of these pressures.
The ‘Hospital at Home’ service has implemented virtual wards allowing patients to receive care at home safely and conveniently. The Hospital at Home workforce includes experienced General Practitioners, advanced clinical practitioners with prescribing rights, registered therapists/nurses, and support workers offering a 7-day service from 8am-8pm.
Main duties of the job
We are looking for hard-working, enthusiastic, highly motivated, and experienced Registered Nurses to join our Hospital at Home team to support this innovative way of working, covering maternity leave.
The primary focus of the role is to provide patient-centred clinical care, encompassing the skills of assessment and diagnosis of patients and providing ongoing acute management under the guidance of the GPs and Advanced Clinical Practitioners within the Hospital at Home team.

Qualifications
* RN Degree / Diploma
* Mentorship and assessing qualification
* Experience within the respiratory, frailty, or community specialities
* Management qualification or equivalent training tool
* Post registration courses including Community Specialist Practitioner course or Level 7 Clinical Assessment and diagnostic modules
Knowledge and experience
* Speciality knowledge and experience
* Knowledge of professional and NHS issues and an understanding of corporate objectives
* Basic IT skills in word processing/Hospital systems
* Experience in a community/Hospital at Home setting
Skills and abilities
* Able to present written information in a logical manner
* Ability to communicate in various settings with patients/staff
* Able to communicate sensitive information
* Ability to develop effective interpersonal relationships with colleagues
* Skills in managing own workload and coordinating the work of others
