Fixed term contract/ secondment opportunity until January 2026
The Inherited Cardiac Conditions Team at NNUH offer an NHS England approved regional service to a population of around 1 million people in Norfolk and extending into Suffolk. The service has close links with the Addenbrookes genetics team, undertaking bimonthly Cardiac Genetics clinics at NNUH. We have close links with the new Barts Heart Centre ICC service.
The recent recruitment of a Sudden Cardiac Death Co-Ordinator post means that you will work alongside this post holder, job sharing the ICC post for 12 months fixed term.

• To provide a comprehensive service for ICC and cardiology patients
• Address remaining geographical gaps by increasing Cardiomyopathy clinic capacity and piloting new service models for cascade screening, to support equitable access to HCM testing in Norfolk and East Anglia
• Provide direct ongoing nursing care to help improve the quality of life, symptom management, patient education and support following diagnosis of an ICC
• Aid transition of patients from paediatric to adult ICC services
• Be a resource for other health care professionals
• Assist in the development of care pathways and packages for patients with an ICC
• Promote a high standard of professional care whilst upholding and influencing professional and trust values

• To operate autonomously at an advanced level of clinical practice providing research based expert care and advice to all colleagues and managing a caseload effectively, making the most of available time
• To manage a specified case load of patients, providing information and support, to enable patients to make informed choices about their care and understand the rational of investigations and treatments.
• To utilise specialist, skills, underpinned by in depth theoretical knowledge, to facilitate the assessment, planning and implementation of individualised programmes of care for patients presenting with a specialist condition and to evaluate their programmes of care.
• To undertake initial assessment of patients including:• physical examination
• assessment/investigation of symptoms
• evaluation of test results
• recommendation of further diagnostic tests
• To work closely with the Consultants, medical and nursing staff, multi-disciplinary team and Heads of Department within the Directorate.
• To ensure that patients are seen within recommended time scales and targets where these are in place.
• To visit inpatients as appropriate to ensure continuity of care and to give advice/ support.
• To accept referrals from medical/nursing staff both within and outside the Trust, of newly diagnosed and existing patients
• To periodically review caseload and to discharge patients appropriately within divisional guidelines
• To respond to telephone enquiries and co-ordinate admission or changes in appointment dates as warranted by the patient’s clinical condition, including operation of a telephone helpline for the relevant speciality if required.
