Job overview
Integrated Well-being Networks have been implemented across Gwent in order to improve well-being through community-based approaches. This programme is part of a whole system transformation to realise the vision of the Welsh Government’s long-term plan for health and social care services.
This exciting new role will lead the development of place-based well-being approaches in Caerphilly county borough. The post holder will continue the development of place-based well-being networks, engaging partners and community assets in a collaborative approach to promoting well-being for all.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The post holder will develop approaches to integrating well-being resources with primary care and community services. The post holder will also build links between the Regional Partnership Board and Public Service Board well-being agendas, working across organisations and service areas.

Qualifications & Knowledge
* Educated to Master’s level or equivalent professional qualification or equivalent demonstrable knowledge and experience of working at a senior level
* Evidence of continuing professional / personal development
* Good understanding of national and local health and social care policy context and drivers
* Good understanding of approaches to community well-being, prevention and early intervention
* Broad knowledge of Assets Based Community Development
* Knowledge of change and quality improvement approaches and methodologies
* Understanding of project management methodologies
Experience
* Significant experience of working in a senior role in health, local government or third sector
* Demonstrable experience of leading, developing and implementing major projects or change programmes
* Demonstrable experience of working successfully with diverse partners across a range of organisations, understanding the “political” context in which they operate
* Experience of Community Development approaches
