The post holder will work as part of the children’s community health commissioned service under the direction and supervision of the SCPHN Health Visitor/School Nurse/Family Health SCPHN/Staff Nurse, contributing to the provision of high-quality services for children, young people, parents, carers, and families working within the framework of the Healthy Child Programme and Best Start In Life priorities. Being proactive in delivering the national and Public Health agendas in response to identified health and social needs, working in partnership with other agencies to meet agreed standards, objectives, and statutory requirements.
Promote the needs of children with SEND, young people with SEND, and their families in the development of community child health services.
To support the SCPHN Health Visitor/School Nurse in the delivery of the Healthy Child Programme, ensuring services are provided within the tiered model approach – universal, universal plus, and universal partnership plus. To be professionally accountable and responsible and adhere to and apply the Trust standards, making every contact count to enable the opportunistic delivery of consistent and concise healthy lifestyle information.
Apply the principles of the Solihull approach in all work undertaken with children, young people, and families.
Apply the Best Start in Life principles where applicable.
Work within family homes delivering packages of care to support families address identified health and development needs.
