Health Monitoring: Observe and document clients' health conditions, reporting any changes to the care coordinator or family when necessary. Transportation Assistance: Accompany clients to medical appointments, grocery shopping, and other essential errands as needed. Care Plan Implementation: Follow individualised care plans to address the client's specific health, safety, and personal needs.
