Main duties of the job
The post holder will be responsible for leading pressure ulcer/wound care management ensuring safe, effective, evidence-based practice in accordance with national/international guidance e.g. NICE NPUAP and EPUAP and local guidance within the Trust wound care/pressure ulcer policies and procedures.
The post holder will be an expert resource for the Trust ensuring seamless care transitioning across inpatient and community services within Airedale, Wharfedale, and Craven and will be expected to display high levels of competence, professional judgement, integrity, and personal accountability. The post holder will improve and maintain standards of patient care through development, implementation, and continuous evaluation of quality improvement initiatives, protocols, and policies. Leading on design and delivery of training and education in both mandatory and bespoke programmes throughout the Trust will be a core responsibility.
The post holder is accountable for their own professional actions and is not directly supervised. They will manage the team to include duty rotas, delegation, PDRs, sickness and absence, and team development opportunities. Developing business plans to embed cost effectiveness whilst ensuring effective service delivery will be a key activity informing service improvement.
Job responsibilities
Leadership
1. Assess current treatments practices maximise cost effectiveness while improving outcomes.
2. Provide expert advice and act as a resource for all aspects of tissue viability, including clinical and professional advice.
3. Design and deliver education and training relating to tissue viability to all levels of staff and professional disciplines.
4. Influence and lead quality care for patients with complex wounds and pressure damage.
5. Lead the Trust review of the wound care formulary and associated products.
6. Support the investigation of acquired pressure ulcers, participating in learning and serious incidents involving pressure ulcers.
7. Provide expert advice to Safeguarding Teams in relation to pressure ulcer incidence and management.
8. Liaise with Clinical Leaders in actioning learning from adverse incidents.
9. Support the effective and efficient management of pressure relieving equipment across the trust in conjunction with MDT.
10. Support the development and implementation of systems for the audit of pressure ulcers and wound care.
11. Dissemination of learning from audit to support best practice.
12. Work with Bradford Community Tissue Viability Team to ensure a coordinated approach to tissue viability services across the acute hospital and community.
13. Liaise with staff working in primary care, hospital, social and caring services, and the voluntary sector as required.
14. Develop effective working relationships with other clinical and non-clinical staff, patients, carers, and relatives.
15. Act as a positive role model and ambassador throughout the trust and beyond by identifying and upholding the value that tissue viability adds to the care of patients.
16. Lead the tissue viability team to ensure effective quality service to patients.
Clinical Practice
1. Act as a clinical role model, which reflects the philosophy, beliefs, and clinical standards of the Trust.
2. Provide a specialist nursing resource in the management of patients with compromised tissue viability, both chronic and acute, and provide advice to other health professionals, patients, carers, and relatives.
3. Make and receive referrals for patients with compromised tissue viability, from nurses, general practitioners, consultants, and other health professionals.
4. Assess, plan, implement, and evaluate patient care utilising advanced knowledge, skills, research, and developments in tissue viability practice.
5. Undertake comprehensive assessments of complex wound management/tissue viability needs.
6. Establish care management plans for staff to implement.
7. Provide education, advice, and support on wound management to staff both inside and outside the trust.
8. Support and promote the use of managed systems for mattresses, ensuring appropriate distribution and use in clinical areas, including the use of E-trace.
9. Provide specialist advice and support relating to the purchasing and commissioning of tissue viability resources and their use in practice.
10. Support the Assistant Director of Nursing and Patient Safety in the identification and monitoring of risks associated with the introduction of new treatments.
11. Influence effective multidisciplinary teamwork and communication to deliver coordinated, seamless care between acute and community services.
12. Promote the interests of Trust at professional meetings and national conferences.
13. Manage and review the stock of specialist tissue viability equipment and dressings.
14. Adhere to Trust infection prevention and uniform policies in the interests of ensuring best practice in infection prevention and control.
Management
1. Promote and maintain a supportive, collaborative team working environment.
2. Contribute to the current and projected service plans identifying gaps and advising management on the level of service provision.
3. Support strategies for wound management contribute to objectives and targets in line with agreed budgets and resources.
4. Manage the budget for specialist dressings and equipment.
5. Improve multidisciplinary collaboration through effective communication with all health care professionals within the multidisciplinary team.
6. Promote and market the activities of the tissue viability service both internally and externally, contributing to the sharing of good practice.
7. Prepare, collate and write reports, as required.
Education and training
1. Identify educational needs of staff, patients, and carers, develop and deliver programs ensuring they are in accordance with the trusts policies, procedures, national practice guidelines, and local standards.
2. Develop and deliver education and training programs as the needs of the service dictate.
3. Establish and maintain links with appropriate academic institutions.
4. Promote and implement evidence-based practice within the clinical environment, according to national and local guidelines, standards, and protocols.
5. Contribute to, and develop income generation opportunities through clinical support and education to external health care organisations and agencies e.g. prison telemedicine.
6. Support and embed a culture of training and education within the tissue viability team.
7. Maintain own practice assurance via clinical supervision and advanced practice framework as defined by the Trust.
Audit and Research
1. Initiate and engage in the Trusts audit program.
2. Lead on audits relating to tissue viability e.g. pressure ulcer prevalence, mattress audits, incidence of acquired pressure ulcers prior to admission etc.
3. Liaise with wards and departments to participate in audits and evaluate clinical practice to ensure quality care.
4. Monitor the implementation of treatment and prevention policies relevant to tissue viability.
5. Utilise audit data and analysis systems to produce reports to support learning and to provide assurance through clinical governance.
6. Liaise and consult with the Assistant Chief Nurse Divisional Directors of Nursing, Clinical Leads and Matrons to develop and design changes in practice following audit and research findings.
7. Actively promote and participate in the Trust risk assessment framework.
Clinical Governance
1. Proactively engage in ongoing audits and research, utilising research findings to ensure patients with tissue viability needs receive evidence-based health care.
2. Ensure nurses are practicing in accordance with national and Trust policies, guidelines, procedures, and standards.
3. Promote evidence-based practice across the Tissue Viability Service.
4. Update Trust Policies and Guidelines, as required.
