Ranworth Primary Care Network have an exciting opportunity for a qualified clinical pharmacist to join our PCN on a full time, permanent basis, Monday to Friday. This role is based at Kennedy Way Medical Centre. The post-holder will work in the p roactive ambulatory team, working to provide expertise in clinical medicines management, medication reviews and repeat prescription authorisation. Duties include: Review the on-going need for medicine of patients facing long-term conditions, monitoring needs and opportunities to support patients with their medicines ensuring they get the best use of their medicines (i.e. medicines optimisation). Undertake patient facing care home clinical medication reviews. Manage caseload of patients with common/minor/self-limiting ailments while working within a scope of practice and limits of competence. Signpost to community pharmacy and referring to GPs or other healthcare professionals where appropriate. Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews. Reconcile medicines following discharge from hospitals, intermediate care and into care homes. Identify and rectify unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge. Provide a telephone help line for patients with questions, queries and concerns about their medicines. Manage the repeat prescribing reauthorisation process. About the Candidate You will be a qualified pharmacist with strong experience within primary care and have the following skills and qualifications: Essential: GPhC registered pharmacist. 2 years working as a pharmacist (or equivalent). Experience in managing pharmacy services in primary care. GPhC independent prescriber qualification. Desirable: CPPE pathway completion preferred. Medicines management qualification. MUR and repeat dispensing certification. Broad knowledge of general practice. Minor ailments certificate.
