Post: Active Life Coach Salary: £26,910.50 per annum Base: Cygnet Hospital Taunton Responsible for: The planning, coordination and delivery of personalised physical activity programmes for individuals within the service Responsible to: Active Life Lead (Line manager) Hours: 40 hours per week Location: Cygnet Hospital Taunton, Orchard Portman, Taunton, Somerset, TA3 7BQ Job Summary Working as part of the MDT. Plan, develop and deliver physical active programs collaboratively with service users (SU) in our care. These healthy life skill programmes provide improvement and fun sessions (IF) supporting SU achieving 25hr of weekly meaningful activity during their stay and will involve some weekend work. In addition developing links and partnerships with local sports facilities, clubs and active partnerships so SU pre discharge are provided with information regarding sessions that take place within their locality so they can continue to enjoy a physical active lifestyle outside of the hospitals active life program. Main duties With line manager plan & deliver physical active programs via one to one’s and group sessions on and off the ward. With patient input. Increase awareness of Active life therapy programme and the benefits of physical activity to all service users, Develop and maintain excellent working relationships across the Hospital including current and future workforces. Support partnership to strengthen active life provision within Cygnet Health Care. Ensuring the highest standers of delivery to meet Cygnet Health Care’s and CQC’s acting values. Provide and record evidence and data daily as and when requested. Attending ward meetings, explaining and promote the benefits of active participation understand this is done with service users at the centre of what we do Support the regional AL lead and other active life team members across all sites & attending CPD and regional development seminar to top up your Active life tool box. Support, record and undertake sufficient promotional work, capture compliments and register for recording and reporting purposes Share information effectively and concisely in routine situations (effective daily documentation; attendance at meetings) face to face handovers of information to colleagues) Work with and update current activity risk assessments, to support individual service line in relation to timetables. Applicable Degree or Higher level equivalent learnt qualification which fits with subject area and post. D1 Driving license Relevant UKCC level 1 or 2 coaching qualifications. Appropriate PMVA, BLS and up first aid Ability to lead groups & 1-1’s in physical active programs.
