Duties :
1. Create individualised care plans for children in care, focusing on their physical, emotional, educational, and social needs to ensure they continue to meet the child's needs.
2. Ensure that the child has access to health, education, and therapeutic services as needed.
3. Liaise with other professionals to ensure services are coordinated and delivered in line with the care plan.
4. Ensure that children are safeguarded from abuse, neglect, or exploitation.
5. Conduct regular assessments of the child's living environment and care arrangements to ensure their safety.
6. Implement risk assessments and act swiftly if any safeguarding concerns arise.
7. Work to ensure that children experience stability in their living arrangements to avoid unnecessary moves or disruptions, which can be traumatic.
8. Monitor placements to ensure that children are settled, supported, and thriving in their environments.
9. Establish and maintain trusting relationships with children in care, providing emotional support and understanding their wishes and feelings.
10. Ensure that children are involved in decisions about their own lives, encouraging their voice to be heard.
11. Support children in understanding their care situation, placement, and future plans, providing age-appropriate information.
12. Work closely with foster carers, residential care staff, or other placement providers to support the child's well-being.
13. Provide guidance and support to foster carers to ensure that placements are stable and meet the child's needs.
14. Resolve any issues or challenges that may arise within the placement.
15. Regularly visit the child in their placement to assess their well-being, identify any concerns, and ensure the placement is meeting their needs.
16. Prepare and submit reports for court proceedings regarding the child's care, including recommendations for long-term plans such as adoption, long-term foster care, or reunification with family.
17. Attend court hearings, providing evidence and ensuring that the child's voice is heard in legal processes.
18. Facilitate and monitor contact between children and their birth families, if it is in the child's best interest. This could include arranging supervised visits or managing indirect contact (e.g., letters).
19. Keep comprehensive, accurate, and up-to-date records of all interactions, decisions, and plans relating to the child's care.
20. Use electronic case management systems to document visits, meetings, assessments, and reviews in a timely manner.
Skills and Requirements :
21. Making informed, child-centred decisions, often in high-pressure situations.
22. Managing multiple cases and meeting statutory deadlines for care planning and reviews.
23. Understanding and empathising with the challenges faced by children in care.
24. Social work degree and registration to social work England
