Main duties of the job
Work closely with all of the paediatricians within the Trust and referring consultants from tertiary centres to provide a comprehensive, high quality service for children who are under the care of a paediatrician at Medway NHS Foundation Trust.

Job responsibilities
* Be a resource within the Medway NHS Foundation Trust and other agencies across Medway and Swale who are involved in caring for this group of children.
* Take part in liaison, teaching and support to all agencies involved in managing children with chronic medical conditions.
* Have experience or willingness to undertake the cannulation & phlebotomy course.
* Be responsible for the provision of competent community nursing support, to meet individual needs of the children, their families/carers.
* Contribute to the quality and development of nursing care in Paediatrics by participating in and organizing teaching programmes, whilst working in partnership with the children and their families and/or carers.
* Assess children's needs and develop and evaluate care plans for an identified patient group, maintaining accurate, contemporaneous records and preserving patient confidentiality.
* Plan and provide specialist nursing care for children who are patients of Medway NHS Foundation Trust in community settings, regularly assessing the care and support that is provided within the home/community.
* Organize home visits for children with chronic medical conditions and provide support for children and their families.
* Encourage parental participation in the care of their child by providing specialist support and practical guidance to these families.
* Provide a supportive and guiding role for families with a terminally ill child at home, participating in a 24-hour on-call rota, if necessary, to support symptom control management.
* Provide and participate in timely training and support to patients, families and staff of outside agencies involved with this patient group.
* Promote and facilitate best practice in all aspects of care delivered to the children and their families and/or carers.
* Be a resource in children's chronic medical conditions, demonstrating skills and expertise to all grades of staff and participating in teaching and assessing programmes.
* Establish a good working environment with all members of the Paediatric Nursing Team in which all children and families receive a high standard of Family Centred Care, maintaining respect and privacy.
* Critically appraise current practice and identify areas for improvement in the light of current research.
* Regularly evaluate and monitor the care delivered to patients using credible systems to ensure it is of the highest standard.
* Ensure that nursing documentation is complete, legible, accurate and relevant, complying with the Nursing and Midwifery Council and the Lord Laming Enquiry recommendations.
* Refer on and have an in-depth understanding of Safeguarding Children and Young People, Child in Need concerns ensuring that local and national guidelines including the London Child Protection procedures are adhered to.
* Have knowledge and understanding of incidents reporting and implementation of Paediatric Risk Management, using the DATIX procedures.
* Facilitate and supervise a supportive learning environment for students and other nursing staff within the Trust.
* Encourage innovation by all members of the team and to personally remain up to date in all matters relating to nursing practice.

Qualifications
* Registered Nurse Child (Child branch or RSCN)
* Degree in children's nursing or evidence of equivalent knowledge and experience
* ENB 998
Knowledge
* Knowledge of the key attributes of nursing children with chronic health conditions in the community is essential for this role
Experience
* Significant general nursing experience, with proven experience as band 5 or 6 in an acute paediatric setting
* Community experience

* Competent and confident in the use of central venous access devices

* Ability to work within a multi-professional team
