About the Role
Sharnbrook Surgery in north Bedford are looking to welcome a senior clinical pharmacist to our team, on a part time basis for 16 hours a week. Our core hours are from 8am to 6.30pm, so the successful candidate will need to be able to work between these times and cover for holidays and sickness as required.
Working within an experienced multi-disciplinary team, this key role will seek to improve patients' health outcomes by providing direct, accessible and timely clinical medicines.
Main duties include:
* Review the ongoing medicine plan for patients with single or multiple problems where medicine optimisation is required (COPD/asthma).
* Undertake clinical medication reviews with patients and produce recommendations for nurses and/or GP.
* Provide patient-facing clinics for those with questions, queries and concerns about their medicines in the practice.
* Provide a telephone helpline for patients with questions, queries and concerns about their medicines.
* Support virtual and remote models of consultation and support including e-consultations, remote medication review and telehealth and telemedicine.
* Respond to medicine related enquiries from GPs, other practice staff, and other healthcare teams.
* Reconcile medicines following discharge from hospitals, intermediate care, and into care homes, working with patients and community pharmacists to ensure patients receive their medicine post-discharge.
About the Candidate
You will be a qualified pharmacist with strong experience within primary care and have the following skills and qualifications:
Essential:
* Mandatory registration with the General Pharmaceutical Council.
* GPhC Independent Prescribing qualification – or working towards attainment.
* Evidence of an ongoing commitment and undertaking of continuing professional development to maintain advanced theoretical and practical therapeutic knowledge.
* Minimum of 2 years post-registration experience in primary care working within a GP surgery is desirable.
* Undertaking, completed or can demonstrate exemption from CPPE’s primary care pharmacy education programme.
* Minimum of 5 years post qualification experience, or equivalent.
* In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
* Excellent interpersonal, influencing and negotiating skills.
* Able to work under pressure and to meet deadlines.
Desirable:
* Membership Primary Care Pharmacy Association (PCPA).
* Membership of the Royal Pharmaceutical Society.
