About us About Our Practice: Our main surgery is located in the heart of Combe Down village on a leafy avenue, just a short walk from the charming Darling Deli - perfect for your morning coffee We also operate a second site at Odd Down in the South of Bath. With nearly 14,000 patients, weve recently transitioned to a total triage model of care, which has received positive feedback from both patients and clinicians. This innovative approach promotes continuity, ensuring patients see the right clinician in an appropriate time frame, whilst also signposting to community services where appropriate. As part of this redesign, we have been able to amend our GP clinics to offer 15-minute face-to-face appointments, interspersed with 10-minute telephone calls and GP-initiated follow-ups. Youll be joining a supportive team of nine GPs, alongside four experienced nurse practitioners. An experienced team of practice nurses conduct the majority of chronic disease management and the recent addition of GPAs has seen a significant reduction in letters and GP administrative tasks. Our Community Role: As the largest member of Minerva PCN, we play a prominent role in local healthcare. The PCN provides a paramedic-run home visiting service, significantly reducing the need for GP home visits to just 1-2 per week per full time clinician. We also have a comprehensive pharmacy team that supports medication reconciliation, complex medication reviews, and HRT/contraception management.
