There will some flexibility as to base location. Main duties of the job We are looking for enthusiastic, proactive and innovative individuals with the ability to think outside the box to join our specialist team of learning disability and autism practitioners and health facilitators. You will need excellent communication and liaison skills in order to engage and collaborate with colleagues, those working in primary care and other stakeholders. The successful candidates will have a strong focus on engagement with patients and their families to enable them to access primary care. The way we care for people with learning disabilities and autism in Suffolk is changing at NSFT as we have recently benefitted from £1.3 million of investment for our acute services and the role out of a new Learning Disability and Autism Specialist Service. The development is fully driven by the national Transforming Care Agenda.
