Provide face to face and telephone consultations for patients presenting with minor ailments and chronic diseases, to establish a diagnosis and management plan using history taking, physical examination, problem solving and clinical decision making Provide home visits to housebound and care home patients to assess and provide appropriate management of acute and chronic conditions Assist the clinicians with shared admin such as workflow, tasks and the review and action of pathology results To prescribe safe, effective and appropriate medication as defined by current guidelines and within own professional competencies Provide care and treatment to patients with chronic diseases through pro-active review and management of the condition focusing closely on individual management to improve health care and compliance Opportunistic health promotion, particularly in relation to diet, exercise, smoking and alcohol. Providing the appropriate preventative health advice and referrals where necessary Provide general and specific health screening within agreed protocols with referral to relevant departments where necessary Work with patients in order to support compliance, and adherence to treatments providing information and advice on prescribed or over-the-counter medication on regimens, side-effects and interactions Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care Assist with vaccination campaigns providing appropriate and timely vaccinations including flu, shingles and covid vaccinations Other Responsibilities within the Practice: Work as an effective and responsible team member, supporting other members of clinical and non-clinical staff in a flexible and approachable manner Awareness of and compliance with all relevant practice policies/guidelines, eg prescribing, confidentiality, data protection and health and safety A commitment to learning and carrying out audits to ensure evidence-based best practise Contributing to evaluation/audit and clinical standard setting within the practice Contributing to the development of computer-based patient records Attending training, practice meetings and events organised by the practice or other agencies, where appropriate Personal / Professional Development Maintain continued education through attendance at any courses and/or study days necessary to ensure that professional development requirements are met Participate in any training programme implemented by the practice as part of this employment, such training to include: Participation in an annual individual performance review, annual Appraisal including taking responsibility for maintaining a record of own personal and/or professional development and to ensure fit for revalidation Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work Maintain own registration with Nursing & Midwifery Council and provide evidence as required Quality Recognise and work within own competence and the professional code of conduct for nurses as set out by the Nursing & Midwifery Council Maintain quality within the practice Alert other team members to issues of Clinical Governance, quality and risk; participate in Significant Event Analysis reviews Deliver care to NICE guidelines, NHS England and ICB policies and evidence-based care Assess own performance and take accountability for own actions, either directly or under supervision Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance Work effectively with individuals in other agencies to meet patients needs Effectively manage own time, workload and resources to contribute to the overall team-working of the practice putting the needs of the practice first Confidentiality In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters.
