We are looking for a staff nurse to work as a member of the team in the Wisdom Hospice to provide holistic nursing care to inpatients with varying conditions including cancer, MND and other neurological conditions, respiratory disease, and heart failure.
This post is for 37.5 hours per week. Option given to work long days or short shifts. The successful applicant will be expected to work rotational days and nights.
You will support senior nurses in planning and evaluating programmes of care in a specialist setting without direct supervision.
You will be triaging out of hours calls from community patients and deputising for the senior nurse when required.

Communication and Relationship Skills
1. Promote a multidisciplinary approach to care by ensuring effective communication is maintained between all staff, patients, and their families.
2. Help guide patients and relatives through the dying process by empathetic listening and being supportive.
3. Attend and present patient profiles at case conferences and multidisciplinary team meetings to ensure effective communications with all members of the Hospice interdisciplinary and primary care teams.
4. Facilitate and encourage effective communication between multidisciplinary teams, participate in relevant meetings, and share knowledge.
5. Support team members who experience a range of emotions from users of service.
Knowledge, Training, and Experience
1. First-level registered nurse.
2. Relevant Post Registration, Palliative Care Course (Palliative Care Dip/Degree, ENB 931, 237) or willingness to undertake.
3. Facilitate the education and support of qualified and unqualified staff and participate in the development of nursing practice on the unit.
Analytical and Judgement Skills
1. Understanding of team working, but also an awareness of the individual needs of each team member.
2. Triage out of hours calls from community patients and refer complex issues to on-call team.
Planning and Organisational Skills
1. Plan and deliver care to patients admitted to the IPU, supported by senior nursing, medical, and wider MDT.
2. Deputise for the senior nurse when required.
3. Motivate and supervise the nursing team in the provision of holistic patient-focused specialised palliative care to patients and carers on the IPU.
Physical Skills
1. Provide physical, emotional, and spiritual support to patients and relatives through excellent evidence-based end of life care.
2. Verify death and provide emotional and spiritual support to patients, families, staff, and other carers to enable them to understand the progression of advanced terminal disease.
3. Safely set up and maintain syringe drivers and administer all medications in accordance with Medway Community Healthcare and NMC guidelines.
4. Participate in integrated internal rotation of all shifts and departments that require nursing skills within the Wisdom Hospice.
Responsibility for Patient / Client Care
1. Manage highly charged emotional situations such as verifying death, conducting after-death sessions, and mortuary duties in a dignified and respectful manner.
Responsibility for Policy and Service Development Implementation
1. Assist Ward Manager and Senior Staff to ensure a safe working environment for patients, relatives, and staff adhering to MCH policies and awareness of legal and ethical responsibilities.
2. Ensure records are kept up to date and patients’ confidentiality safeguarded in accordance with the organisation's policies and NMC.
3. Keep up to date with all statutory and mandatory training keeping an up-to-date record of attendance through a learning passport and ESR.
Responsibilities for Financial and Physical Resources
1. Assist nurse in charge with on-site responsibility for security, catering, portering, and domestic duties of unit out of hours.
Responsibilities for Human Resources (HR)
1. Regularly attend Clinical Supervision provided for MCH staff.
2. Act as a mentor for students.
Responsibilities for Information Resources
1. Link nurse for specific areas of palliative nursing care.
2. Maintain patient confidentiality.
Responsibilities for Research and Development (R&D)
1. Maintain own clinical and professional competency, attend educational sessions provided within the Hospice and externally.
2. Identify learning and development needs in the appraisal process.
Freedom to Act
1. Discretionary control and administration of prescribed ranges of medication.
