Job Summary:
1. This is an exciting opportunity for a registered Social Worker to join a small, dynamic team who will concentrate on completing outstanding reviews for adults receiving care and support.
2. To assess and care manage the complex needs of adults and their carers to ensure their health, wellbeing and social inclusion.
3. To liaise with Reablement and other professionals to ensure seamless care pathways which safeguard and promote independence.
4. To promote partnership working which supports the person-centred outcomes to be delivered for older people.
5. To undertake safeguarding investigations as required.
Local Authorities meet their statutory obligations under Mental Health Law and the Social Services and Wellbeing (Wales) Act 2014 and others.
Key Duties/Accountabilities (Sample):
1. The needs of adults and their carers are assessed holistically, packages of care negotiated, monitored and reviewed within the first six weeks.
2. Maintaining a small but very complex caseload.
3. Risks to older people and others are assessed and balanced in a way that promotes independence of service users and their carers.
4. Recommendations about allocating scarce resources are made.
5. Equal opportunity and respect for diversity and difference is promoted.
6. Social inclusion of isolated and vulnerable people is promoted.
7. Abuse of vulnerable adults is investigated.
8. Collaboration and multi-disciplinary working with other agencies is strengthened.
9. Accountable for actions through effective, accurate recording of decisions in line with policy and legislation.
10. Safeguarding the welfare of a vulnerable person at immediate risk of harm.
11. Engaging in direct work with older people and carer/s to carry out integrated assessment for older people.
12. In working with older people and carers, Social Workers seek to enhance their problem-solving capacities in a way that support maximum independence and choice.
13. Considering with older people and carers options to best meet assessed eligible needs and assist in making informed decisions.
14. Undertaking Mental Capacity Act assessments and Best Interests decisions alongside advocates.
15. Adopting a creative and holistic approach whilst exploring options.
16. Seeking to maximise the financial and material resources available to service users from all possible sources.
17. Recording unmet need.
18. Collating the findings of the assessment and completing assessment documentation in accordance with statutory guidance, legislation and departmental procedures and timescales.
19. Preparing court and other specialist reports in the required format.
20. Ensuring case file recording and all relevant documentation is completed in accordance with departmental policy and procedures.
21. Manage conflict effectively within individual, family and professional situations.
22. Use professional assertiveness to justify decisions and uphold professional social work practice.
23. Appropriately challenge judgements and decisions of others where there is evidence that the needs of those you are working with are not being met.
24. Convening and/or attending interagency meetings/ liaison e.g. case conferences.
25. Obtaining managerial/panel agreement for personal plan of care delivery.
26. Recording and disseminating care plan in accordance with statutory requirements, legislation and departmental procedures.
27. As well as individual caseloads, having an important role to play both directly and indirectly in shaping services.
28. Understand budget and resource implications in constructing high quality and value for money care provision.
29. Encourage service users where appropriate to consider positive risk taking to move forward with their lives.
30. Delegating monitoring task (if appropriate) to another individual e.g. reviewing officer, care provider following six week review.
31. Arranging and co-ordinating review of the Care Plan for small ongoing caseload.
32. Engaging directly with older people / families / carers / in the review of the compiles Care Plan and agreeing changes where necessary.
33. Negotiating and agreeing changes to plan with providers and other agencies involved.
34. Ensuring that older people are aware of complaints procedures and advocacy services.
35. Contributing to the professional development of others by acting as a mentor for newly qualified practitioners undertaking professional training following first year in practice.
36. Timely administration of all aspects of caseload Preparing and engaging in professional supervision with Line Manager.
37. Establishing and maintaining good working relationships with colleagues.
38. Identifying and addressing areas for professional development and training.
39. Complying with Welsh language requirements of the position - no competency level.
40. Adhere to health and safety advice and guidance.

43. This position has a requirement for an Enhanced DBS and ISA registration as it is classed as Regulated Activity under the Vetting and Barring Guidance, 2009.
44. This post is exempt from the Rehabilitation of Offenders Act.
45. Demonstrating Continual Professional Development including meeting the requirements of the Care Council for Wales for continued registration as a Social Worker.
46. Sharing skills, knowledge and learning with colleagues in other specialisms.
47. Continue to meet the requirements of the national occupational standards in social work and adhere to the code of practice for social workers.
48. Undertake post-qualifying training which include practice assessor module, best interest assessor, AMHP or post qualifying assessing.
49. Use a range of methods at appropriate times to seek feedback on your performance from service users, carers, colleagues, managers and mentors in order to reflect on your practice.
50. Prepare for and engage in practice observations by your line manager or Case Work Manager.
51. Contribute to team /peer learning programmes.
52. Strive to be an effective team player.
53. Record and agree with your manager examples of your progress and learning in order to meet re registration requirements.
Skills/Experience:
1. 12 months post qualifying experience of working with Older People and their and carers.
2. Experience of undertaking assessments of need and identifying and managing risks.
3. Experience of complex casework.
4. Experience of care management across the range of health and social care settings.
5. Understanding of the needs of Older People and their carers.
6. Understanding of safeguarding and the issues affecting Older People and their carers.
7. Knowledge of current legislation, national guidance and best practice relevant to the post.
8. Knowledge and implementation of the Mental Capacity Act.
Qualifications & Training:
1. Appropriate social work qualification (i.e. Degree in Social Work, DipSW or equivalent).
2. Registration as Social Worker with CCW/CQC.
3. Commitment to undertake appropriate CPEL qualification.
4. Good communication skills, written and verbal.
5. Good IT skills.
6. Ability to organise, prioritise and manage a caseload.
7. Ability to build and maintain partnerships.
8. Ability to manage crisis, conflict resolution and challenging behaviours.
9. Ability to demonstrate a commitment to equal opportunities and non-discriminatory practice.
10. Commitment to the principles of confidentiality.
11. Must have knowledge and commitment to the principles of social care as embodied in relevant legislation.
Additional Information:
1. Full time role: 37 Hr/week from 8.30 to 16.45.
2. Candidates must hold a professional qualification in Social Work, such as a diploma in Social Work, BA/BSc/MA in Social Work, or CQSW.
3. Candidates should be able or willing to learn, how to pronounce personal and place names in Welsh.
4. Current driving licence or ability to travel extensively around the county.
