Job summary This is a new role in Anfield and Everton PCN, enabling one lead coordinator to focus exclusively on the work we undertake to reduce inequalities, in one of the most deprived areas in England. Working closely with the PCN Manager, you will help to plan the inequalities programme for the DES and manage initiatives, events, communication and staff accordingly. Anfield and Everton PCN has a very strong history of delivery in this respect, especially around breast screening and supporting complex patients, and we plan to expand those strengths to more and more inequalities areas. Main duties of the job To support the PCN Manager to plan the inequalities programme for the PCN Direct Enhanced Service -DES-, or equivalent and additional initiatives. To lead the delivery of the agreed DES programme, supporting the PCN Manager to lead the care coordinator and social prescriber teams to ensure safe and effective care and support to reduce inequalities is delivered to the population of Anfield and Everton PCN. To plan and deliver inequalities and associated patient and staff events. To represent the PCN at meetings relevant to the inequalities and associated agendas, and report progress to the PCN Manager. To develop reports about the work undertaken and support their analysis. To work closely with the PCN Administrator to ensure effective communication of inequalities work planned and undertaken. To supervise the care coordinators and social prescribers and support the PCN Manager to create and monitor their workplans and development plans To provide project management support to the PCN Manager, working on defined projects, including monthly reporting and delivery of quality improvement initiatives To demonstrate effective leadership and communication skills, whilst working within relevant legal and ethical frameworks and in accordance with organisational protocols and policies.
