Job summary The community staff nurse role would include holistic assessment of patient care needs and development, implementation and evaluation of evidence care. Duties include palliative care support and assessment, care at the end of life, supporting and assessing individuals and carers with long-term health conditions, promotion of self-management skills and wound care. Main duties of the job The post holder will be responsible for autonomous evidence based safe, delivery of care and treatments in patient homes within the community, in line with competency and training record and organisational policies, procedures and guidelines. Assessing, planning, implementing and evaluating care without direct supervision. To work in partnership with patients, carers, to offer support, advice and promote informed choice. Working an collaborating with the multidisciplinary team to provide gold standard care. Support the role of the community nurse team leader.
