An Opportunity for a part-time MDT Coordinator and Governance support has arisen in the Cardiac Sciences Directorate at Imperial College Healthcare NHS Trust.
The current work pattern is Monday to Wednesday but that might change in the future, depending on service needs.
As part of the role, the candidate will be working with our Multi-Disciplinary team to ensure that MDT meetings run effectively and are well structured in order to improve patient pathways and flows within the Cardiac Department. The candidate will also support the directorate governance structure by providing admin support to the team in the form of database updates and document management.
The candidate will be expected to demonstrate excellent administrative and managerial skills, good communication and ability to organise and prioritise their own and their team workload.
Our world-class cardiology service at Hammersmith Hospital is a tertiary referral cardiac centre led by academic specialists who are leaders in their field. We use cutting-edge clinical innovations to treat and prevent heart conditions and offer a full range of investigations, treatments and management for cardiac conditions. We provide cardiac assessment and treatment for a broad range of patients, including specialist clinics for patients with end-stage kidney disease, as well as offer a national pulmonary hypertension service. Although our cardiology service is primarily based at Hammersmith Hospital, we also offer our services at St Mary’s and Charing Cross hospitals.
The post holder will be mainly based at Hammersmith hospital but might be required to occasionally travel across sites.
• To support the relevant Lead Clinician and multidisciplinary team in identifying patients for discussion at the weekly Clinical Review meetings. Collating and presenting patient data (including patient proformas, summary sheets, investigation reports) and distributing this to all team members in a timely fashion to facilitate full discussion.
• Ensure attendance at all ward-rounds and MDT meetings are recorded.
• To ensure all documentation, case notes, etc are available for the weekly Clinical review meetings (which may also involve liaison with other hospitals) so that a full/informed clinical decision can be made. To maintain attendance records for these meetings. Following the meeting, updating the action plan and circulating as instructed.
• To ensure any Clinical correspondence is completed under the instruction of the Consultant of the week.
• To assist in the design and collection of audit data, in order to measure service efficiency and implemented changes.
• To specifically enter the NICOR data for the cardiology unit.
• To ensure accurate record of the outcomes of the MDT meetings.
• Contribute to the Peer Review process by providing required audit data (e.g. MDT attendance data)
