An opportunity has arisen for a Community Mental Health Nurse to join our team in Middlesbrough at Woodside Resource Centre. This post will involve working as part of our memory service and your role will involve holding a caseload of service users who have concerns about memory, completing assessment and diagnostic support following our dementia pathway.
Nursing applicants should already hold NMC registration and have successfully completed their preceptorship programme. Please note: Newly qualified Nurses or those due to qualify will be recruited via a separate process.
Main duties of the job
As a community practitioner you will have a genuine interest in collaborative working with older adults experiencing functional or organic mental health problems including their carers and families.
As a registered nurse this is an ideal opportunity for someone who wants to develop community clinical skills. As a core member of the team, you will also be responsible for your own caseload, providing assessments, care co-ordination and delivering intervention in the community with people who have complex needs ranging from mild to moderate level of need to severe and enduring level of need and complexity. You will demonstrate excellent assessment skills, interpersonal, and team working skills and be able to work collaboratively with a range of statutory and third sector agencies.
You will have an enthusiastic approach towards your own personal and professional development. In return, you will receive regular appraisal, clinical and management supervision.
We also provide monthly inhouse education sessions to improve core skills and develop our understanding of service need.
