Job overview
An exciting opportunity has arisen for an enthusiastic Community Practitioner to join the Kolvin Service. The Kolvin Service comprises a regional community adolescent forensic mental health service, a nationally commissioned FCAMHS and mental health and forensic services into two Secure Children’s Homes (SCHs) in the North East for young people aged 10-18 years.
The Kolvin Service collaboratively leads on the implementation of the SECURE STAIRS trauma-informed framework at Aycliffe Secure Centre and Kyloe House SCHs. The SECURE STAIRS framework represents a shift in focus within the secure estate from interventions focused solely at young people, to interventions focused on the whole system.
The challenging nature of this post requires suitably applicants to be an experienced mental health or learning disability nurse who is highly motivated, enthusiastic and well organized, have excellent communication skills, sound clinical assessment, risk management and intervention skills; and a commitment to multi-disciplinary working is essential. The successful candidate will have the opportunity to learn and develop skills delivering therapeutic work and trauma-informed care as well as being able to demonstrate the capacity to understand the impact of working with trauma on professionals and carers.
Main duties of the job
1. Assist the MDT in the service delivery, be responsible for carrying out comprehensive mental health assessment including risk assessments, management plans and care plans and support coordinating the provision of therapeutic interventions with young people.
2. Support the MDT providing a service for children and young people with complex trauma, who may be considered to present high risk to themselves, high risk to others or are highly vulnerable.
3. Offer assessments, carry out interventions, and support professionals and carers working with the children and young people to support the principles of the SECURE STAIRS Framework.
4. Consider Carer needs and ensure Carer and family are involved in Care Planning where appropriate.
5. Deliver evidenced based Clinical and Psychologically based Interventions.
6. Provide compassionate care that is based on empathy, kindness, respect and dignity.
7. Incorporate a client centred philosophy into nursing practice and client centred collaborative care.
Person specification
Education and Qualifications
* Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist.
* Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
* Proven track record of post registration clinical experience relevant to the post.
* Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
* Up-to-date knowledge/experience of relevant legislation.
* Evidence of CPD and evidence based practice.
Experience and Knowledge
* 12 to 24 month band 5 qualified nurse experience dependent upon service need.
* Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.
* Understanding of difficult to manage/challenging behaviours.
Skills and Competencies
* Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
* Risk Assessment and Risk Management / Safety Planning skills.
* Demonstrate an understanding of effective & appropriate treatment options including a range of evidence based clinical approaches as identified in N.I.C.E. Guidance.
* Demonstrate effective written and verbal communication skills.
* Evidence of a high level of professional, organisational and leadership skills (self & others).
* Ability to adapt within a changing environment.
* Knowledge of RIO and Data Systems.
* Knowledge and Application of psychologically based interventions.
