Job summary The Band 7 role is based within the Southampton Primary Care Network in GP surgeries. It is focused on taking day to day responsibility for delivering a primary care based clinical operations approach that better integrates primary care services and mental health support to improve health and wellbeing outcomes in a high utilising patient group. The clinical approach will seek to meet the on the day demand offering triage, assessment, formulation and maximising brief psychological intervention. The successful applicant will become part of a growing primary care team who deliver care to some of the most diverse populations in Southampton. Main duties of the job The team leader role is largely clinical; you will contribute to the triage service; offering early assessment, brief interventions and signposting for people with mental health needs in their local communities. You will play an active part in the development of systems and processes which support care delivery within these teams. You will require good communication skills and be able to build positive partnerships with key stakeholders in primary and secondary care. The PCN is a large, inclusive team with a focus on multi disciplinary working. There is an intention for mental health team to develop. The work is varied and challenging, and there is a strong team approach ensuring good support for the successful applicant.
