Our therapy team works across multiple locations in Solihull, providing vital services to individuals who need further assessment and rehabilitation after being placed in discharge-to-assess beds or returning home for continued care. We focus on patients with complex discharge planning needs following hospital admissions. This role involves providing physiotherapy services in patients' homes and working closely with community services including the community therapy team. The role also involves being part of the respiratory on-call list for UHB hospitals. Working hours are 37.5 per week, with shifts including 7-3, 8-4, and 10-6, covering weekends and bank holidays. To support our patients in their recovery journey and minimise the risk of hospital readmissions. To make the transition from acute care to the community as smooth as possible, ensuring timely discharges and efficient flow. To liaise with various agencies to ensure safe, effective and efficient discharges. To provide Physiotherapy services in the patient's own home or residence.
