As a Band 6 Community Practitioner, you will coordinate mental health care and treatment for patients within our locality area with cognitive and functionally frail conditions, focusing on care in the community. You will provide thorough assessment and treatment plans within NICE guidelines to a caseload of patients across Sunderland. You will be able to work autonomously and as part of a team.
Our Older Adults Pathway is a multi-disciplinary team with other functions available which can support complex patient needs such as our Day Hospital, Step Up Service, Behaviour Support Service, Physical health monitoring, and Care home liaison team.
It is expected that you will possess excellent communication and interpersonal skills and have the ability to manage your time effectively.

Our service is currently provided from Monday to Friday, 9 am - 5 pm (excluding Bank Holidays), with some out-of-hours flexibility from 8 am - 8 pm to meet individual needs.
Main Duties of the Job
* Be responsible for the assessment and management of casework of service users who have substantial and complex needs.
* In collaboration with the service user and carer where appropriate, develop formulation-focused care plans utilizing a psychosocial approach, ensuring timely reviews to monitor progress and adapt care plans as necessary.
* Provide compassionate care that is based on empathy, kindness, respect, and dignity.
* Provide clinical and psychologically-based interventions.
* Provide specialist advice to service users, carers, staff, statutory agencies, third sector partner agencies, and the public.
* Provide clinical supervision to staff as delegated by the Clinical Lead and Pathway Manager.

Person Specification
* RMN or equivalent.
* Experience working in a community setting with Older Adults.
* Knowledge and awareness of Care Coordination.
* Ability to work within the MDT and with other agencies.
* Good communication skills.
* Awareness of safeguarding procedures.
* Ability to meet the mobility requirements for the post.
