Staff Nurse - Tenbury Community Hospital

Tenbury Community Hospital is a rural inpatient setting of 22 beds that supports patients as a step down to receive ongoing medical treatment, rehabilitation, discharge planning or end of life care which no longer requires acute support but cannot be managed in the community.

Main duties of the job
As a Staff Nurse, you will be responsible for providing care to people admitted to our ward for rehabilitation. We work closely as a multi-disciplinary team. Our focus is promoting the comfort and wellbeing of patients, enabling them to live as independently as possible. Tenbury is a small but friendly team to work with. As a team, we encourage ongoing learning and development through reflection and clinical supervision which we try and embed in our daily practice. Our nurses are required to facilitate the day-to-day running of the ward, administering medications, working alongside all members of the MDT as well as patients, their families, and carers to provide 24/7 rehabilitation to enable the highest level of independence and quality of life. Our nurses are required to attend meetings as needed such as MDT and CHC or Discharge Planning.

* Responsible for the assessment, planning, implementation and evaluation of patients' care within the Ward/unit, working in accordance with the philosophy of patient-centred care and to NSF and other Trust standards.
* Practice as a member of the Community Hospital Nursing team, to support and work in partnership with the wider multidisciplinary team.
* Planning and Delivery of Individual Nursing Care under the Direction of Senior Staff.
* Work within the NMC Code of Professional Conduct.
* Assess, plan, implement and evaluate patient-focused nursing care.
* Document care given and maintain contemporaneous records.
* Maintain records in accordance with the Trust's policy and that of NMC Code of Professional Conduct.
* Assist in the implementation of health promotion strategies, as agreed by Ward Manager.
* In consultation with senior qualified staff, provide and monitor patient-centred care within Trust agreed standards.
* In collaboration with other team members, organise the reception and discharge of patients ensuring effective communication in the transfer of information.
* Carry out delegated nursing care.
* Use and participate in research/audit in care delivery.
* Adhere to and promote all clinical practice, health and safety, and corporate Trust Policies and Guidelines.
Person Specification
Qualifications
* RGN/RMN
* NMC Registered
* Practice Supervisor and/or Practice Assessor Training.
* Accredited courses in Palliative Care, Tissue Viability, Diabetes, Dementia.
Experience
* Experience gained during pre-registration training.
* Experience gained working in the frailty care setting.
Knowledge
* Confidence to teach student nurses, patients, carers and members of the multidisciplinary team.
* Experience of Audit.
* Knowledge and understanding of Mental Capacity and DOLs.

Salary: £29,970 to £36,483 a year pro rata for part time.
