We are recruiting a Care Home Matron to work in Rural North Chichester, to work in close partnership with Primary Care Network (PCN), care homes, and local care providers.
Main duties of the job
These are well-established roles, ideal for enthusiastic, autonomous, self-motivated, confident, and proactive practitioners. The successful candidates will work as key components of an integrated team, liaising with our PCN and care home partners, with a focus on delivering the PCN Directed Enhanced Service: Enhanced Health in Care Homes specification.
The post holders will need to link with wider SCFT services, forming strong links both internally with our Community Nursing teams and externally with the wider PCN teams, care homes, and medicine optimisation services.
They will undertake advanced clinical assessment, needs identification, planning, case management, and coordination of care for individuals with highly complex needs and co-existing long-term conditions. They will work proactively to reduce unplanned hospital admissions and facilitate discharge from hospital for care home residents identified through weekly check-in meetings with the care homes and the dedicated PCN team.
