Job Overview
Specialty Doctor Community Medicine
12 month fixed term contract - possibility to extend
This is a full-time fixed term 12-month post in Community Medicine with potential to extend beyond 12 months by mutual agreement. Applicants able to work 3 – 5 days per week and willing to job share will be considered. The post holder will be part of a dynamic and experienced team of community clinicians. The role incorporates practice of General Internal Medicine, Gerontology, Rehabilitation, Long term conditions, and Palliative care.
The post is held within the Adult and Urgent Community Response division (A&UCR) of Bromley Healthcare (BHC) and provides an excellent opportunity for doctors who want to gain experience working in community medicine. It will involve placement within the Hospital at Home service with opportunity for experience with the Rapid Response and Rehabilitation services.
Additionally, there is close working with the Acute Frailty Assessment Unit at the Princess Royal University Hospital, Community Palliative services provided by St Christophers, and with the Bromley GP Alliance. There is scope to incorporate experience with One Bromley’s Proactive pathway (providing ‘at home’ care planning and case management for complex patients, often with frailty). This post is suitable for Specialist trainees looking for an Out of Programme Experience (OOPE).
Main Duties of the Job
The post holder will work across different areas of care where they will develop competencies in assessing and treating patients in the community. Time spent in each area will be tailored according to patient flow, in order to optimise learning opportunities. The post holder will have training opportunities within BHC and is expected to keep a training portfolio. They will be provided with appropriate clinical and educational supervision.

* Teaching - Experience of teaching medical & allied health professionals.
