Job overview
Note: You must have a valid full UK driving licence and full access to a car (Monday - Friday) which is insured for the purposes of work, to be eligible to take up this post.
A new opportunity has arisen to join the MFT Alcohol Care Team (ACT). This is an exciting time to join the team, as it expands and develops within the hospitals and the wider trust.
The Supportive Outreach and Engagement Worker position has been created to support individuals accessing secondary emergency care services, providing a wider, holistic and multi-agency approach to case management.
This role will be primarily community-based; however, you may at times be requested to work at one of the 3 Alcohol Care Team locations: Manchester Royal Infirmary, Wythenshawe Hospital, or North Manchester General Hospital.
Main duties of the job
The focus of the role would be working with high intensity users of the trust, who present to the Emergency Departments or are admitted in relation to alcohol misuse. You will ensure effective implementation of care pathways and be the interface between community substance misuse services and the Alcohol Care Teams. You will provide the extension of care beyond discharge, supporting individuals who may be resistant to services, hard to engage, or treatment naïve at the point of discharge. The post holder will attempt to re-engage individuals who have dropped out of treatment or detached from services.
