Care Coordinator role
Band 3 - 25 Hours per week, Permanent
We are looking for a Ward Coordinator to join our team in patient flow. The successful candidate will have a determination and a keen ability to help our patients and staff to maintain the adequate flow of patients to enable our patient to be treated and discharged in a timely manner.
You will liaise with the medical and nursing teams to facilitate safe and appropriate patient discharges.
Working alongside the nursing team you will assist in patient referrals, assessments, and taking admissions from the emergency department and internal departments, you will also undertake minimal patient audits.
This role will involve working alongside IUCT and the discharge team to facilitate discharges and maintain patient safety.

The post holder acts as the first point of contact for all forms of communication for the Nurse in Charge of the ward and their team.
Ensures all discharge/care plans are current and up to date working with the named nurse and MDTs
To provide clerical/ and general support to the ward/departmental team.
Generally assisting in a non-nursing capacity as directed by the nurse in charge i.e. assist as need be with meals and patients beverages as required.
