Advanced Practitioner Practice Development – Health Interface Service (Adults)

The Health Interface Service (HIS) works to both prevent admissions into the acute hospitals, finding quick solutions to support people to remain at home as well as supporting people upon their discharge from an acute hospital. We have developed an innovative front door admission avoidance service that supports people return home that do not require an acute admission. Join us and you will be working in one of the most exciting areas of an acute hospital, where decision making, and action is fast paced but robust and person centered.
We are looking for people willing to work across our countywide service - we have staff working in the acute hospitals that cover Somerset, predominantly Yeovil District Hospital and Musgrove Hospital in Taunton, but we also provide cover to Weston General and Royal United in Bath. You would need to be prepared to travel to each area at times.
The Advanced Practitioner - Practitioner Development is a brand-new role, you will have the opportunity to join a network of Practice Development Advanced Practitioners within Somerset. These posts are based in operational teams which will keep you close to practice.
Your day-to-day work will involve:
* Providing high quality practice education for students and NQSWs.
* Supervising up to 3 registered social workers.
* Embedding practice improvement activities and learning from reviews into teams.
* Promoting a strong social work framework that incorporates relationship-based practice, research, co-production and legal literacy.
We offer ongoing support, training and guidance to help you be the best you can be. But to be successful in the role you will:
* Need to be a qualified Social Worker, registered with Social Work England, with experience of working in Adult Social Care. It is essential that you either hold PEPS 1 and 2 or are willing to work towards these qualifications on appointment.
* Need to be a driver and have access to a vehicle as some parts of rural Somerset are not on direct transport links.
* Have commitment to high standards of professional social work practice.
* Have the knowledge and skills to link theory, social work models, research and legal frameworks to practice.
* Have the ability to hold reflective spaces, challenge constructively and influence others.
* Be committed to ensuring that the voices of people with lived experience are heard throughout your work.
In turn, you will receive regular supervision and be part of a developing network of Practice Development Leads in Somerset. You will develop and maintain strong links and working relationships with Learning and Development and the Principal Social Worker alongside having access to bespoke training to enable you to be successful in these roles.
This is a Grade 9 post with a current salary of £40,476 - £44,711 per annum. Appointments are normally made at the bottom of the salary scale.
