The post holder will provide specialist nursing care, advice, guidance and leadership, and take responsibility for a specialist nursing service using their advanced clinical knowledge and experience with a high level of skill and theoretical knowledge to:
1. Assess, plan, implement and evaluate evidence-based nursing care.
2. Provide expert advice and clinical practice.
3. Work autonomously and manage own workload as a lead specialist in their area.
4. Provide specialist education and training to staff.
5. Lead and manage a team of specialist nurses.
6. Work collaboratively and co-operatively with clinical colleagues to develop services and the quality of care delivered.
7. Lead the development and planning of clinical services within the specialist field.
8. Demonstrate the Health Board's values and ensure that others demonstrate the same values.
9. Undertake clinical activity e.g. clinics, ward-based work, telephone consultations etc.
10. Undertake professional activity e.g. audit, CPD, teaching, research and administration.
Main duties of the job
To work as a Nurse Practitioner within a multidisciplinary team setting providing rapid health and social assessment, care and support to patients, carers and families in their own home in order to avoid unnecessary hospital admission and in turn promote patient independence. To use professional judgement and advanced clinical skills to initiate assessment, diagnosis and the implementation of a range of care interventions for patients presenting with undifferentiated and undiagnosed problems.
Job responsibilities
The Post holder will be expected to exercise a high degree of personal professional autonomy and make critical judgments to satisfy the expectations and demands of the role.
1. Assessment, examination, diagnosis and treatment of patients on the case load.
2. Monitoring of bloods, prescribing of treatments, ensuring as a Practice Leader that standards and Health Board Policies are maintained at all times.
3. Working with other departments/agencies to provide improved outcomes for patients by improving timelines and consistency of care.
4. The ability to travel.
