We are looking to appoint a Level 2 or Level 3 Social Worker to join the Hospital Team based at Huddersfield Royal Infirmary, Acre Street, Lindley, Huddersfield, HD3 3EA.
You will have the opportunity to provide invaluable support and aid to those who need it most. This is a chance to truly make a difference in the lives of others.
You will be part of a busy team which works closely with the multi-disciplinary team to facilitate timely and safe discharges from hospital.
Key Responsibilities:
1. Responsible for the day-to-day management of your assigned cases.
2. Undertake strength-based assessments of individuals and their carers, to establish eligibility for support under the Care Act.
3. Work effectively as part of a team and with a range of agencies and professionals.
4. Complete positive risk assessments and safeguard adults at risk.
Requirements for the role:
1. Social Work qualification and be SWE registered.
2. Full and valid driving licence.
3. Experience of working with vulnerable adults and their carers.

The contract will be a 5 over 7-day contract, working on a rota basis with the team. You will receive a 1.81% enhancement of your pay to cover weekends and Bank Holiday working.
