Job summary The role is central to the co-ordination of both complex and non-complexdischarges. This may include complex multi-disciplinaryassessment signposting, liaising directly with patients and their families/Care Transfer Coordinators/ MDT's assisting in case conferences forpatients with complex health and social care, palliative care orrehabilitation or housing needs. Giving families support and information. Support the patient flow hub, as well as working closely with a range ofprofessionals and external agencies within the hospital and in communityservices. Work independently, following procedures and treatment plans, but under the direction of the patient flow hub, and within agreed boundaries. Responsible for ongoing/complex planning in regards to process mapping patients journeys for modelling purposes. Provide training on systems to new staff joining the team.
