What you will do: As a member of the team you will work closely with the wards and other departments to ensure effective flow throughout the hospital site. You will work closely with the Discharge Hub staff, and other partnerships to ensure a safe and efficient discharge pathway to the community. With minimal supervision and in collaboration with the multi-disciplinary team, the post holder will be accountable for the assessment, development, implementation and evaluation of programmes of care for a defined caseload of patients working within the professional standards held in the NMC Code of Professional Conduct.
