KMPT is Seeking a Dedicated Community Psychiatric Nurse

We are seeking a highly motivated and compassionate Community Psychiatric Nurse to join our multidisciplinary team.
You will play a crucial role in providing holistic care to individuals with diverse mental health needs within the community.
Key Responsibilities:
* Conduct thorough holistic assessments of individuals' mental and physical health needs, social circumstances, and support systems.
* Develop, implement, and evaluate personalized care plans in collaboration with service users and the multidisciplinary team.
* Coordinate and deliver a range of therapeutic interventions, medication management, and practical support.
* Monitor and evaluate the effectiveness of care plans, making adjustments as needed.
Main duties of the job:
* Arranging, contributing to and documenting care reviews, including telephone, home visits, video and face to face reviews - as appropriate and defined within the care group SOP.
* To supervise junior staff members and to support students in placement.
* To work collaboratively with the multidisciplinary team as well as patients, carers, family members and internal and external agencies.
* Effective communication skills and self-governance are required. You will be able to demonstrate knowledge of high-level interventions & community assessment skills. Existing qualifications or a willingness to undertake training relevant to the post is expected.

Qualifications
* Essential criteria:
o Registered with NMC as an RMN.
* Desirable criteria:
o Experience working as RMN with people who has mental health difficulties.
* Essential criteria:
o Graduate.
* Desirable criteria:
o Post Graduate.
