What the Job Involves The role involves assessing the needs of adults with mental health concerns and their informal carers in accordance with the Care Act and eligibility criteria. You will work collaboratively with health and social care colleagues to devise strengths-based support plans, facilitate integrated approaches, and provide seamless support to service users. Responsibilities include conducting risk assessments, managing caseloads, implementing support plans, and contributing to service development. You will maintain accurate records, develop partnerships with multiple sectors, and ensure efficient service delivery through new technologies.
