The Older Adult Community Mental Health Team is situated in Enfield, which is close to amenities and good transport links. We are a dynamic, friendly and supportive integrated team that consists of Social Workers, Occupational Therapists and Care Co-ordinators. Our team provide support and services to older adults of Enfield who live with functional and organic mental health disorders. The role involves working in the community as well as working across our mental health wards at Chase Farm Hospital. Our work is focused around changing and improving lives. You will work alongside individuals, their families/ carers, health and other partners, using a person-centered and strengths-based approach, to facilitate timely and proportionate responses to need, considering their views, abilities and resources. You will work with individuals and their carers who may require social care services, by establishing their social care and health needs. You will work with them to plan, implement and review actions and outcomes, whilst at all times working towards giving a greater choice, promoting independence and social inclusion. Safeguarding is Everybody's Business' and we work closely with our Multi-Agency Safeguarding Hub (MASH) to ensure the protection of adults at risk. You will be required to complete s42 Enquires and Safeguarding Adults Reviews, in line with the Care Act 2014 and our Safeguarding Adults procedures. You will need to have sound knowledge and understanding of the Care Act 2014, and experience in completing person centred and strengths-based assessments; implementing personal budgets; developing Support Plans; Mental Capacity Assessment; Safeguarding Adults process and procedures; and Continuing Health Care assessments.
