We are looking to recruit a Registered Nurse with both Clinical and Management experience, who will have responsibility for the day-to-day management and clinical leadership of the Urgent Community Response & Virtual Ward team across East Berkshire.
The role involves working closely with service users, community-based services, Primary care, other community health services and social services, and the voluntary sector to ensure that the services provided are of high quality, responsive to local need and demonstrate value for money.
You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards. You will ensure good communication, and effective processes and procedures are maintained across the service.
The post holder will work 37.5 hours per week, 7.5 hours a day, Monday to Friday between the hours of 8 am - 8 pm.
The role is based at St Marks Hospital and free staff parking is available.
Main duties of the job
1. To practice at an advanced level of nursing, working as an autonomous practitioner within the multidisciplinary team including admitting to and discharging patients from the Urgent Community Response Service Virtual Frailty Ward in collaboration with the UCR VFW Geriatrician and Specialty Doctor.
2. As an experienced nurse practitioner, you will take responsibility and be accountable for the clinical assessment, diagnosis, treatment and management of complex patients considering local and national guidelines.
3. The postholder is expected to liaise effectively and professionally with all levels of staff, external organisations and the public, dealing with confidential information in an appropriate manner.
4. To provide leadership and line management to the Band 8/7/6 nurses and therapists and the Band 4 healthcare/multi-therapy assistants.
5. Ensure accurate, contemporaneous documentation and care plans are maintained in accordance with the NMC, information governance and local/national guidance.
6. Work collaboratively with colleagues and develop nursing and care delivery policies, identifying how policy will impact on practice and service delivery.
7. Work flexibly in response to service need across our multiple sites.
