The Role
An opportunity has arisen for a Community Assessment Officer within the Intermediate Tier Services. This service covers Admission Avoidance, Intermediate Care and Discharge to Assess bed-based units.
This post offers an opportunity to contribute to the ongoing development of innovative ways of working across health and social care in order to improve the experience of service users and carers and to promote well-being and independence.
The ability to develop, maintain and improve relationships with service users and carers is essential in this role as well as the attributes required to participate in multi-disciplinary team working.
About You
We are looking to recruit a dedicated and motivated individual who is passionate about working with adults, understanding their needs, aspirations and supporting them to enhance their quality of life and promote independence.
As a Community Assessment Officer in Intermediate Tier Services you will work in one of the designated services and undertake assessments/reviews of need under the Care Act 2014, assist with support planning as necessary and also link people into universal and voluntary services. You will assist colleagues within other areas of the services as necessary.
You will have excellent communication and interpersonal skills, organisational skills and be able to work well in a multi-disciplinary team.
