An exciting opportunity has arisen for an experienced Diabetes Specialist Nurse to join our existing team, providing Adult Diabetes Services across the Havering area for two days per week.
Our Integrated service model encompasses a multidisciplinary approach to Diabetes Care in the community setting, allowing shared responsibility for the person with diabetes. The service aims to maximise the opportunity for preventing complications of diabetes through an effective empowerment and partnership approach, focusing on individual care planning, support for professional and patient education and emotional and psychological care.
Main duties of the job
The main responsibilities of the post include:
1. Working within a team of skilled diabetes specialist nurses to deliver a high standard of evidence-based clinical care to patients and their carers.
2. Delivering education sessions to both patients and healthcare professionals.
3. Exhibiting excellent communication skills and confidence to work alone and within a multidisciplinary team.
4. Contributing to the development of our multi-disciplinary specialist community service.
5. Committing to continued personal development and being able to travel efficiently across the area of Havering, including to patients in their own homes.
About us
Starting with NELFT: NELFT places great importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training, and the allocation of equipment. New starters will have the opportunity to meet the executive team, senior managers, and attend drop-in sessions focusing on engagement, health and wellbeing, and key processes. The induction will be held at our head office in Rainham, Essex.

High Cost Area Supplement - London: This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (minimum payment of £4,313 and a maximum payment of £5,436 per annum pro rata).

* Proficient in IT skills including MS Office applications, Virtual platforms & Electronic Patient records.
