As well as enjoying their home and being part of everyday living, each person loves getting out in their local communities to do the things they like and need.

Minimum Requirements:
Manual driving licence required.

Your responsibilities:
* To empower the people we support to live the lives they want to lead.
* To develop daily living skills.
* Support people to access their communities for socialisation, employment, training or friendships.
* To contribute to, develop and implement the individuals' care plans.
* To communicate effectively and professionally with people we support, their family members, and external professionals.
* Maintain the health needs of the individuals we support.
* Completion of a dementia/end of life 12-week course.
* A full, manual driving licence is essential.
